3. He must have a reasonable leave record. 4. The department director must have advanced the maximum amount of sick leave that he is authorized, 80 hours for a full time employee. part time (PT) employees must have been advanced by the department director the maximum amount of sick leave that he is authorized as follows: PT 20, forty (40) hours; PT 25, fifty (50) hours; or PT 30, sixty (60) hours. C. A benefit eligible employee who has at least five continuous years of service who has not exhausted City Manager’s leave may be eligible for donated leave for the care of a sick family member if they meet all the requirements as outlined in 5.3 of this policy. See the City Manager’s Leave policy on page 196 for more information. 6.0 PROCEDURES 6.1 Employee’s Personal Illness A. An employee is expected to notify his supervisor when he will be out of work for an extended period of time due to a serious illness or injury if physically able to do so. B. When the supervisor becomes aware of the serious, extended absence, he should review the employee’s leave record and determine if the employee is eligible for donated leave and if so, when. C. If the employee has met the basic qualifying requirements as outlined in 5.3 of this policy, the supervisor should forward a recommendation to request donated leave to his department director. This should be done at least two weeks prior to the exhaustion of all eligible accrued paid leave. D. If the supervisor is unsure of the employee’s eligibility for donated leave, he should discuss this with management of his department and/or the designated human resources department contact for this policy. E. The department director will review the recommendation of the supervisor, the employee’s leave record, information concerning expected return to work, and if appropriate will recommend, in writing, to the human resources director that donated leave be requested for the employee. F. Normally the department director of the employee needing the donated leave will make the request for donated leave (with the prior approval of the human resources director) within the department prior to a City-wide request. If the department’s donated leave is insufficient to cover the absence, the human resources director may make a city-wide request for additional donated leave. G. Donations received will be sent to the human resources benefits consultant for compilation. H. The human resources department will monitor the donated leave balance. The supervisor
