Optum Live and Work Well Employee Support Program Number: H-5 (former Employee Assistance Program) Revision: 6 Effective Date: 05-01-2023 1.0 POLICY T he City of Greensboro provides the Optum Live and Work Well Employee Support Program (LAWW) in order to assist employees and their families in the early intervention of any personal problems which may affect the employee’s job performance. 2.0 PURPOSE T he City values each employees’ personal wellbeing. In an effort to help each employee be more successful, the City provides the Optum LAWW Employee Support Program to assist employees to resolve personal problems. The City wants to assist employees through this voluntary and confidential service but in no way does this policy or program excuse an employee from the expectation of adequate job performance. 3.0 SCOPE This policy applies to all employees. 4.0 DEFINITIONS 4.1 Eligible Employees - All employees including rosters are eligible for coverage and participation in the City’s benefit programs. 4.2 I mmediate Family or Household Member - For the purpose of this policy, “immediate family” is defined as the employee’s spouse and children. “Household member” is defined as any member of the employee’s household. 4.3 Optum Live and Work Well Employee Support Program (LAWW) - A workplace benefit program that offers confidential support for managing stress, anxiety and depression; parenting and family needs; workplace concerns; sleep issues; and substance abuse. 4.4 Confidentiality - Information and all records related to services rendered by Optum LAWW providers and subsequent referrals are kept confidential in accordance with federal and state laws. 4.5 Consent - For the purposes of this policy, consent means express written permission to share, collect or otherwise have access to pertinent personal medical information for the purpose of performing a complete health evaluation.
