not able to determine that a health issue exists in order to qualify for FMLA job protection. In this event, the absence may be classified as unauthorized leave and subject to the corrective action policy. 6.4 When the FMLA coordinator notifies the department that the time-off requested qualifies for FMLA protection, the supervisor/manager must go into the City’s time-keeping system and classify the time taken as FMLA using the appropriate code. 6.5 At least five working days before the employee’s scheduled return to work, the employee must contact their supervisor to inform them of the employee’s intent to return to work. The employee must also contact City Medical Services to schedule a Fitness for Duty evaluation and a Return to Work evaluation only if the FMLA usage was related to his/her own illness. 7.0 HUMAN RESOURCES CONTACT Benefits Consultant Medical Services 8.0 APPENDIX, APPENDICES FMLA Leave Request Form
