document, Flexible Work Arrangements Evaluation Factors . 6.4 At a minimum, the FWA agreement must address: the employee’s work hours; core hours; work assignments; expected results; the provision and use of equipment; required supervisor contact; and liability for personal or property injury. 6.5 The finalized agreements will be maintained by the department. 6.6 FWA agreements may only be modified in writing and with approval from the employee’s supervisor, Department Director and when applicable the executive leader. 6.7 Departments may terminate FWA agreements with a two-week notice to the employee.
