Coverage • There will be no waiting period for pre-existing conditions for new hires and eligible dependents on the Choice Plan or the Choice Plus Plan in-network, effective January 1, 1998. • The Choice Plus Plan out-of-network has a 12 month waiting period for pre-existing conditions, unless a certificate of prior coverage is provided. • There is no waiting period for employees and eligible dependents enrolled in the dental plan when first eligible. • Late enrollees will have a 12 month waiting period for any orthodonture treatments. • Waiting periods will be reduced or eliminated if proof is provided showing all of the following cri- teria have been met: - Greensboro Police Department”s Crime Analysis Unit loves working for the City of Greensboro because they get to wear red, promote wellness, and DAB with some cool people. • Individual must have been covered under a health and/or dental plan. • Loss of coverage must have occurred on or after October 1, 1996. • The break in insurance coverage is no more than 63 days.
