AMTA Coverage AMTA Members are covered for professional services for which the enrolled member is licensed, certified, accredited or professionally trained to perform as a massage therapist. Student Enrolled Members are covered only for those services for which the Student Enrolled Member is professionally trained to perform while engaged in school sanctioned and directed activities. If an enrolled member practices in any jurisdiction which governs massage therapy services, then massage therapy services means those services for which the enrolled member is licensed, certified, accredited, trained or qualified to perform within the scope of practice recognized by the governmental regulatory agency responsible for maintaining the standards of the profession of massage therapy. Professional services also means the enrolled member's massage therapy services while acting as a member of a formal accreditation, standards review, or similar professional board or committee, including the directives of such board or committee. As an AMTA enrolled member covered by the AMTA insurance program, enrolled members are responsible for and expected at all times to be familiar and current with all laws, regulations, etc. in their state of practice that govern their profession as a massage therapist. Modality Exclusions Any acts, errors or omissions involving the activities designated below are excluded. This list is subject to review and change by AMTA. Electrolysis or microcurrent; radiation for the removal of hair; photocoagulation technique for the removal of hair; plastic surgery of any type; removal of warts, moles, or other growths; laser or other types of pulsated light treatments; weight reducing treatments; sun tanning treatments; Botox; hair implanting or hair transplanting; tinting, dyeing or coloring of hair, eyelashes, or eyebrows; removal of any form of permanent cosmetic makeup; yoga trapeze, yoga swing, or aerial yoga hammock; surgical or non surgical body contouring; or flotation or sensory deprivation pod therapy. Colon hydrotherapy, nutritional or dietary counseling, personal training, pilates, religious healing, procedures that use fire, cupping therapy with use of heat, ear candling, saunas, sun tanning treatments other than topical tanning lotions or sprays, procedures which penetrate the skin or body cavities either manually or with other methods of intrusion other than manual soft tissue manipulation of the oral or nasal cavities. Diagnosis, prescription, or service in the capacity of any other profession or branch of healthcare or medicine for which a license to practice is required by law including chiropractic, dentistry, dermatology, naprapathy, naturopathy, nursing, orthopedics, osteopathy, physical therapy, podiatry, psychiatry, psychology or psychotherapy. This Nonparticipating Dentist Fee may be less than what your dentist charges, which means that you will be responsible for the difference. The explanation and sample calculation of how these services will be paid can be found in Section VI – How Payment is Made in your Certificate.  Oral exams (including evaluations by a specialist) are payable twice per calendar year.  Prophylaxes (cleanings) are payable twice per calendar year.  Space maintainers are Covered Services without limitations.  Bitewing X-rays are payable twice per calendar year and full mouth X-rays (which include bitewing X-rays) or a panorex are payable once in any three-year period.  Sealants are payable once per tooth per three-year period for primary and permanent molars and bicuspids for people age 15 and under. The surface must be free from decay and restorations.  Veneers are payable on incisors, cuspids, and bicuspids once per tooth per five-year period when necessary due to fracture or decay.  Composite resin (white) restorations are payable on posterior teeth.  Inlays (any material) are Covered Services.  Porcelain and resin facings on bridges and crowns are Covered Services on posterior teeth.  Coronal splinting on natural teeth or prosthetic crowns is a Covered Service.  Vestibuloplasty, biopsy of hard tissue, and surgical excision of soft tissue and intra-osseous lesions are Covered Services.  Implants are payable once per tooth in any five-year period. Implant related services are Covered Services.  Crowns over implants are payable once per tooth in any five-year period. Services related to crowns over implants are Covered Services.  Nitrous oxide and antibiotic drug injections are Covered Services. Occlusal guards are payable without limitation.  People with special health care needs may be eligible for additional services including exams, hygiene visits, dental case management, and silver diamine fluoride treatment. Special health care needs includes any physical, developmental, mental, sensory, behavioral, cognitive, or emotional impairment or limiting condition that requires medical management, healthcare intervention, and/or use of specialized services or programs. The condition may be congenital, developmental, or acquired through disease, trauma, or environmental cause and may impose limitations in performing daily self-maintenance activities or substantial limitations. Passport Dental Having Delta Dental coverage makes it easy for you to get dental care almost everywhere in the world! You can now receive expert dental care when you are outside of the United States through our Passport Dental program. This program gives you access to a worldwide network of Dentists and dental clinics. English-speaking operators are available around the clock to answer questions and help you schedule care. For more information, check our website or contact your benefits representative to get a copy of our Passport Dental information sheet. Delta Dental PPO™ Dentist or Delta Dental Premier® Dentist - $1,750 per person total per Benefit Year on all services except orthodontics. $2,400 per person total per lifetime on orthodontic services. Nonparticipating Dentist - $1,250 per person total per Benefit Year on all services except orthodontics. $2,400 per person for Dependent Children only per lifetime on orthodontic services. These are not separate maximums by type of dentist. You must use a Delta Dental PPO or Delta Dental Premier Dentists in order to access the $1,750 Maximum Benefits payable. Payment for Orthodontic Service – When orthodontic treatment begins, your Dentist will submit a payment plan to Delta Dental based upon your projected course of treatment. In accordance with the agreed upon payment plan, Delta Dental will make an initial payment to you or your Participating Dentist equal to Delta Dental's stated Copayment on 30% of the Maximum Payment for Orthodontic Services as set forth in this Summary of Dental Plan Benefits. Delta Dental will make additional payments as follows: Delta Dental will pay 50% of the per month fee charged by your Dentist based upon the agreed upon payment plan provided by Delta Dental to your Dentist. Nonparticipating Dentist - $50 Deductible per Member total per Benefit Year limited to a maximum Deductible of $100 per family per Benefit Year on all services. Waiting Period – Enrollees who are eligible for Benefits are covered as defined by the plan administrator. Eligible People – All Active, Retiree and Cobra participants who meet the requirements of the Plan Administrator that elect coverage. The Subscriber pays the full cost of this plan. Also eligible are your Spouse and your Children to the end of the month in which they turn 26, including your Children who are married, who no longer live with you, who are not your Dependents for Federal income tax purposes, and/or who are not permanently disabled. Enrollees and dependents choosing this plan are required to remain enrolled for a minimum of 12 months. Should an Enrollee or Dependent choose to drop coverage after that time, he or she may not re-enroll prior to the date on which 12 months have elapsed. Healthy Bento Beach Boxes

Makes 8 Servings Or 40 appetizer rolls

1 cup Roasted red pepper hummus ½ cup Feta crumbles 8 Low carb or whole-wheat tortillas 1 Red bell pepper, cut in sticks

✓ Bento boxes provide a great lunch option for the beach or pool with minimum clean up or mess. ✓ Veggie and pastas salads make great healthier choice to chips. ✓ Wraps can be made the morning before your trip and wrapped in parchment or wax paper to prevent them from becoming soggy. ✓ Minimize prep by buying precut vegetables like baby carrots, snow peas, mini peppers, broccoli, or cherry tomatoes. ✓ Pack healthy trail mixes or raw nuts for a healthy snack.

1. Spread hummus one the middle of the tortilla leaving edges empty. Top with 1 Tablespoon of feta. 2. Add a thin layer of the vegetables across two thirds of each tortilla. Fold sides in and fold bottom up and tightly roll to the top. 3. Slice into about 5 individual rolls per tortilla. Secure each roll with a toothpick.

½ cup Red onion, sliced thin ¼ cup Fresh Parsley, chopped 1 teaspoon Dried Dill weed ½ cup Red Wine Vinaigrette dressing

1. Combine all salad ingredients except dressing, mix thoroughly. 2. Pour dressing over mixture and toss to combine. Chill for at least two hours before serving to let flavors develop.

1. Slice a watermelon into one-inch rounds. 2. Cut each watermelon round into fourths or eighths depending on the size of the watermelon. 3. With a sharp knife, cut a small slit into the center of the green rind, but not as deep as the red juicy flesh of the watermelon. 4. Insert a large craft stick through each slit and push into the watermelon until only about 3 inches of the stick is visible from the bottom. 5.
