We understand that not knowing the cost of care can cause stress for members. That’s why we created a more transparent, personalized and empowering pricing tool. Members can use it in the moment and as a proactive resource to help estimate and manage health care costs.

Here’s how it works:

Members enter their visit or procedure type or choose it from the list. They can also search by a specific provider or find a provider offering a service within their area. If they’re getting surgery, for certain services this tool will look at their total care journey, such as X-rays, physical therapy and follow-up appointments. It will estimate how much they’ll pay based on their health plan, including whether their deductible has been met for the year.

Orthodontic Age Limit – Adult - In Network Coverage only * When you receive services from a Nonparticipating Dentist, the percentages in this column indicate the portion of Delta Dental's Nonparticipating Dentist Fee that will be paid for those services. This Nonparticipating Dentist Fee may be less than what your dentist charges, which means that you will be responsible for the difference.  Space maintainers are Covered Services without limitations.  Bitewing X-rays are payable twice per calendar year and full mouth X-rays (which include bitewing X-rays) or a panorex are payable once in any three-year period.  Sealants are payable once per tooth per three-year period for primary and permanent molars and bicuspids for people age 15 and under. The surface must be free from decay and restorations.  Veneers are payable on incisors, cuspids, and bicuspids once per tooth per five-year period when necessary due to fracture or decay.  Composite resin (white) restorations are payable on posterior teeth.  Inlays (any material) are Covered Services.  Porcelain and resin facings on bridges and crowns are Covered Services on posterior teeth.  Coronal splinting on natural teeth or prosthetic crowns is a Covered Service.  Vestibuloplasty, biopsy of hard tissue, and surgical excision of soft tissue and intra-osseous lesions are Covered Services.  Implants are payable once per tooth in any five-year period. Implant related services are Covered Services.  Crowns over implants are payable once per tooth in any five-year period. Services related to crowns over implants are Covered Services.  Nitrous oxide and antibiotic drug injections are Covered Services. Occlusal guards are payable without limitation.  People with special health care needs may be eligible for additional services including exams, hygiene visits, dental case management, and silver diamine fluoride treatment. Special health care needs includes any physical, developmental, mental, sensory, behavioral, cognitive, or emotional impairment or limiting condition that requires medical management, healthcare intervention, and/or use of specialized services or programs. The condition may be congenital, developmental, or acquired through disease, trauma, or environmental cause and may impose limitations in performing daily self-maintenance activities or substantial limitations. Passport Dental Having Delta Dental coverage makes it easy for you to get dental care almost everywhere in the world! You can now receive expert dental care when you are outside of the United States through our Passport Dental program. This program gives you access to a worldwide network of Dentists and dental clinics. English-speaking operators are available around the clock to answer questions and help you schedule care. For more information, check our website or contact your benefits representative to get a copy of our Passport Dental information sheet. Delta Dental PPO™ Dentist or Delta Dental Premier® Dentist - $1,750 per person total per Benefit Year on all services except orthodontics. $2,400 per person total per lifetime on orthodontic services. Nonparticipating Dentist - $1,250 per person total per Benefit Year on all services except orthodontics. $2,400 per person for Dependent Children only per lifetime on orthodontic services. These are not separate maximums by type of dentist. You must use a Delta Dental PPO or Delta Dental Premier Dentists in order to access the $1,750 Maximum Benefits payable. Payment for Orthodontic Service – When orthodontic treatment begins, your Dentist will submit a payment plan to Delta Dental based upon your projected course of treatment. In accordance with the agreed upon payment plan, Delta Dental will make an initial payment to you or your Participating Dentist equal to Delta Dental's stated Copayment on 30% of the Maximum Payment for Orthodontic Services as set forth in this Summary of Dental Plan Benefits. Delta Dental will make additional payments as follows: Delta Dental will pay 50% of the per month fee charged by your Dentist based upon the agreed upon payment plan provided by Delta Dental to your Dentist. Nonparticipating Dentist - $50 Deductible per Member total per Benefit Year limited to a maximum Deductible of $100 per family per Benefit Year on all services. Waiting Period – Enrollees who are eligible for Benefits are covered as defined by the plan administrator. Eligible People – All Active, Retiree and Cobra participants who meet the requirements of the Plan Administrator that elect coverage. The Subscriber pays the full cost of this plan. Also eligible are your Spouse and your Children to the end of the month in which they turn 26, including your Children who are married, who no longer live with you, who are not your Dependents for Federal income tax purposes, and/or who are not permanently disabled. Enrollees and dependents choosing this plan are required to remain enrolled for a minimum of 12 months. Should an Enrollee or Dependent choose to drop coverage after that time, he or she may not re-enroll prior to the date on which 12 months have elapsed. Aromatically: Using an essential oil aromatically means experiencing its properties through the air. Aromatic usage typically includes breathing in or inhaling an essential oil, allowing the scent to interact with your senses. Diffusing is powerful, supporting any/all of the 10 systems of the body within 20 seconds, elevating mood, and/or killing airborne pathogens. Topically: Applying an essential oil topically is a safe, effective way to enjoy the benefits it offers the skin. When diluting the essential oils, use a carrier oil at a ratio of 1:1. It is not necessary to dilute all essential oils, but for some essential oils which are particularly cool (Peppermint, Wintergreen) or hot (Oregano, Cinnamon, Clove, Cassia), it may be more comfortable to dilute. An effective option is to combine a few drops of essential oil with a small amount of carrier oil and apply at the soles of the feet. Do not use essential oils on an open sore or wound. To support the healing process once the wound begins to heal, dilute with a carrier oil, and apply an inch or so around the affected area. For pain and digestive support, rub the oil(s) right on the spot of discomfort, or on the soles of the feet. Ten Tips for Managing Prediabetes

1. Reduce Your Weight. Research shows that reducing your body weight by 5 to 7 percent – 10 to 14 pound for someone who weighs 200 pounds – may reduce the conversion to full blown diabetes by 58%. If you have been recently been diagnosed as pre-diabetic, you can reverse the trend by following these tips and utilizing the resources available. 2. Exercise. Find something you enjoy. Try a daily walk with friends or colleagues, group exercise class or YouTube exercise videos. As little as 15 minutes of exercise at a time can add up. Always check with your doctor before you start a program. Exercise lowers blood pressure, stress and bad cholesterol and may improved energy, circulation and strengthen your heart, muscles and bones. 3. Get Enough Sleep. Quality sleep is an essential part of maintaining good health. A lack of sleep can lead to changes in hormone levels including insulin and cortisol, leading to increased blood sugar levels and weight gain. 4. Eat the Right Foods. Increase fruits, vegetables, whole grains, lean proteins, healthy fats and low-fat dairy. CHOOSE WATER By replacing soda, alcohol or fancy coffee drinks with water, you can cut a significant number of calories from your daily routine. Many people can lose weight by simply replacing these high sugar beverages with water throughout the day. Also try drinking water before your meal. The water will help break down the food more effectively.

7. Eat More Often. Eat Breakfast – Research shows that people that eat breakfast weigh less than those that skip the meal. Snacking- Have a healthy snack try fresh fruit, fresh raw vegetables, a hand full of raw nuts (about ¼ cup), or 1 ounce of low-fat cheese. 8. Stop Drinking Your Calories . Did you know that 47 % of added sugar consumption in the US comes from sweetened beverages? General recommendations are to drink 8 to 10 glasses of water a day. (This equals 64 to 80 ounces). Fill two 32-ounce containers each morning and drink by dinner time. Add fresh oranges, lemons or cucumbers to flavor your water. 9. Plan Ahead When Dining Out . Review the menu online. Ask how the item is prepared and if you can substitute. Avoid jumbo, giant, deluxe or super size . Avoid battered, breaded, fried crunchy. Double up on your non starchy vegetables. Ask for vegetables without butter or sauces. Ask for sauces and dressings on the side. Ask for a to go container when you place your order. Put half your meal in t as soon as the meal arrives for another meal. Share a dessert so you only consume half the calories. 10.
